The Pennsylvania Interscholastic Athletic Association (PIAA) has developed an all sport pre participation physical form that must be completed prior to any sport season. Once this form is completed, no recertification will be necessary unless an injury has occurred and the athlete cannot complete the current season or has missed 25% of the season; therefore, a recertification must take place before starting the next sport season.
