INFORMATION FOR OBTAINING A WORKING PERMIT

STEP 1
To obtain an application for a working permit, a parent or guardian must appear in person to Mrs. Sullivan in the Guidance Office. The parent/guardian must bring some official document, i.e birth certificate, baptismal certificate, passport, etc. with the student's birth date. The parent/guardian must sign the application in Mrs. Sullivan's presence.

STEP 2
The student takes the application to the employer who completes one part of the application and to a physician who completes another part of the application. When the application is completed, the student must return the application and sign the working permit in Mrs. Sullivan's presence. If the student is 14 or 15, the working permit is mailed to the employer. If the student is 16 or 17, the student is given the working permit, which is a transferable one that is good for any jobs he/she obtains until he/she is 18 or graduates.
